How to Obtain Working Papers during COVID-19 School Closure

Port Chester students ages 14-17 who wish to obtain working papers during COVID-19 school closure should:

 
1. Complete the following application, parts 1 and 2 only with STUDENT Information http://www.p12.nysed.gov/sss/documents/AT17-0320Fillable.pdf
 
2. A parent must sign where indicated in part 1
 
3. An up to date physical from your doctor from the past year (must be dated after 4/16/19) is needed and if it is on file with the health office, that one will be used. If not or if expired, you will need to provide one.
 
4. Email completed application and physical (if not on file with the school) to:  workingpapersPCHS@portchesterschools.org
 
Once received, your application will be processed and your work permit will be mailed to your home address with instructions on where the student should sign. To avoid delay, please make sure your application is legible and includes STUDENT'S information, not the parent's. A parent's signature is the only item required.
